*Service Level Statement: "Assistance in Addressing Malnutrition in Children Under 5 Years (Service ID: 19072709100)"
1) Introduction:
The program to address malnutrition in children under 5 years old has been a collaborative support initiative since 2006, in partnership with the Ministry of Health, Treatment and Medical Education, the Ministry of Cooperatives, Labor and Social Welfare, and the Organization for Planning and Budget. It began as a pilot project in three provinces and, after evaluation and positive effectiveness results, was expanded to seven provinces and then nationwide.
2) Objectives of the Statement:
- Support families and the youth population.
- Address malnutrition in children under 5 years (59 months).
- Prevent the onset of various communicable and non-communicable diseases.
- Alleviate the crisis of malnutrition among vulnerable segments of society.
3) Definition of Service:
The Imam Khomeini Relief Committee, in collaboration with the Ministry of Health, Treatment and Medical Education, and the Organization for Planning and Budget, distributes food baskets in the form of non-cash vouchers to address the malnutrition of program beneficiaries.
4) Interaction with Service Recipients:
Identification and referral of mothers are conducted by health and treatment centers to the relief committee. Following eligibility assessment, necessary actions for data registration and voucher issuance are taken. The program's budget is provided by the Organization for Planning and Budget, and vouchers are charged at least every two months with an amount of 1,250,000 IRR.
5) Reporting and Review Process:
A child suffering from malnutrition is covered by the program from six months of age and can receive up to 18 food baskets (until the end of two years). A malnourished child receives a six-month supply of six food baskets starting at 12 months of age, and if malnutrition persists, they receive another six-month supply before exiting the program.

- Identification and referral of malnourished children under 5 years by health and treatment centers to the provincial offices of the relief committee.
- Eligibility assessment of beneficiaries based on means testing by the relief committee.
- Charging non-cash vouchers at least every two months by the Relief Fund.
8) Mutual Commitments of Service Recipients and Executive Body:
- Charging non-cash vouchers.
- Providing food baskets by the service recipient.
9) Clear and Efficient Process for Resolving Disputes:
In case of non-charging of vouchers for various reasons, a supplementary list is prepared to compensate for the lack of service, and charging is carried out.
10) Costs and Payments:
Funds are deposited by the Organization for Planning and Budget into the relief committee's account, and the per capita food basket is deposited into the non-cash voucher account at least every two months.
